Assignment Title: SOAP Chart Notes
What I learned: In this assignment I learned how to utilize and fill out a Nutrition Assessment Form using SOAP Chart notes. Currently both ADIME and SOAP Charting notes are used for nutrition assessment. These nutrition assessments forms consists of Anthropometric and Biochemical measures, Physical Evaluation, and Dietary History. To fulfill these assignments I learned how to understand lab values, medicinal side effects, weight, dietary intake and the connections they may make with the disease states or underlying nutrition issues.
Reflection to assignment: SOAP Chart Notes are critical components to assessing patients when they have been admitted to a medical care facility. Learning how to utilize these forms can be challenging and sometimes patients do not follow the etiology that is expected from specific illness or disease. Keeping an open mind with assessment and diagnosis and really listening to patients/clients is key.Relevance to the profession: The skills learned for these Nutrition Assessments are key components to providing the best care for patients / clients to help them heal and provide comfort. These assessments help clarify the much need calories and nutrients that may be lacking for a patient in need.
